Vivos Provider Consent and Compliance Agreement
This agreement outlines the guidelines for accessing and using Vivos educational materials, including case reviews, creative assets, and patient images.
1. Purpose of Use
The educational materials provided by Vivos are strictly for educational purposes within the provider's professional role. They must not be used for marketing, advertising, or any commercial activities unless explicitly authorized in writing by Vivos.
2. Restrictions on Use
No Recordings: I agree not to record, capture, reproduce, or distribute any part of the case reviews by any means, including audio/video recordings or screen captures.
Patient Image Restrictions: I will not use any patient images for marketing or other purposes without the explicit written consent of the patient or their legal guardian.
Ownership and Rights: Vivos retains all rights, title, and interest in the materials provided. I agree to use the materials solely within the educational context. Any other usage requires prior written permission from Vivos.
Modifications: I may only replace Vivos-provided patient images with those from my own practice. For any other modifications, I will seek prior authorization from Vivos.
3. Educational Use Agreement
I understand and acknowledge that all case reviews and educational content are provided exclusively for training purposes. I agree not to share, distribute, or utilize the materials beyond their intended educational use.
4. Review Publication
By submitting a testimonial or review, I understand that it may be published on The Vivos Institute’s website or social media. This may include my name, photo, professional title, and practice name/location.

5. Compliance with Agreement
I acknowledge that failure to comply with the terms of this agreement may result in immediate revocation of access to materials, along with potential legal consequences.
